Each LEC shall keep a record containing the following information for each customer account:

1. (1) The name of the customer for whom the deposit is made;
2. (2) The service location;
3. (3) The deposit amount;
4. (4) The deposit date;
5. (5) The reason for the deposit;
6. (6) Each transaction concerning each deposit; and
7. (7) All efforts to return the deposit to the customer.
