Each water utility shall maintain a record of the flushing of its mains to include the following information:

1. (1) Date of the flushing;
2. (2) Location of the flushing;
3. (3) Name of the utility employee performing the flushing;
4. (4) Duration of the flushing; and
5. (5) Colors of the flow during the flushing.
