1. (a) Each electric utility shall maintain accurate records of trouble reports.
2. (b) Each record shall be maintained for at least two (2) years.

3. (c) At a minimum, each record shall include:

   1. (1) Appropriate identification of the:

      1. (A) Person or persons reporting the trouble; and
      2. (B) Specific geographic area;
   2. (2) Time and date of the initial report;
   3. (3) Description of the trouble reported;
   4. (4) Description of the trouble found by the electric utility;
   5. (5) Action taken to clear the trouble; and
   6. (6) Date and time the trouble was cleared.
