An application for a CCN by a telecommunications provider other than an LEC shall include:

1. (1) The name and address of the telecommunications provider and:

   1. (A) The full name of its owner if a sole proprietorship and of each partner if a partnership; or
   2. (B) A full list of the officers and directors if a corporation;

2. (2) A copy of the:

   1. (A) Articles of a partnership or a certified copy of the articles of incorporation or other articles of organization; and
   2. (B) Certificate of authority of a foreign corporation unless incorporated under Arkansas law;
3. (3) A description of the telecommunications provider and the telecommunications service or services it proposes to provide;
4. (4) A description of the telecommunications provider’s technical means to provide the proposed service;
5. (5) Information setting forth the financial condition of the telecommunications provider;
6. (6) An initial tariff or price list proposed for the services to be offered;

7. (7)

   1. (A) A detailed description of the transmission facilities whether owned or leased, switching equipment, and the points at which the transmission facilities or switching equipment connect to the local exchange network through switched, special, or private access service, together with a description of the type of access service used to connect to the local exchange network.
   2. (B) This same information shall be provided with regard to direct access to any customer; and
8. (8) Such other information as the Arkansas Public Service Commission may require.
