(b) If a casino licensee surrenders its license, the casino licensee shall:

1. (1) Return the license to the commission;

2. (2) Submit a report to the commission including:

   1. (A) The reason for surrendering the license;
   2. (B) Contact information following the close of business;
   3. (C) Person or persons responsible for the close of the business; and
   4. (D) Where business records will be retained.
