(b)

1. (1) The list shall be distributed to every franchise holder in the State of Arkansas on at least a quarterly basis via email or United States Postal Service.
2. (2) Each franchise holder shall acknowledge receipt thereof by notifying the commission via email or correspondence.

(c) The following information and data shall be provided for each excluded person:

1. (1) The full name and aliases of the person to be excluded;

2. (2) A description of the person’s physical appearance, including:

   1. (A) Height;
   2. (B) Weight;
   3. (C) Type build;
   4. (D) Color of hair and eyes; and
   5. (E) Other physical characteristics which might assist in the identification of the person;
3. (3) Date of birth;
4. (4) The effective date of the order of exclusion; and
5. (5) A photograph, if obtainable, and the date of the photo or a photo taken by the franchise holder’s surveillance department.
