Applications to the State Board of Embalmers, Funeral Directors, Cemeteries, and Burial Services for an agent’s license must be:

1. (1) Filled out;
2. (2) Signed before a notary public by the applicant and an officer of the association; and
3. (3) Accompanied by the statutory fee.
