(b) The request for approval shall include:

1. (1) A comprehensive description of the proposed change that sets forth:

   1. (A) The identity of all persons acquiring control under the proposed change; and
   2. (B) The ownership interest and managerial authority of all persons in control under the proposed change; and

2. (2) For each new person in control under the proposed change:

   1. (A) Biographical information, including employment history for the immediate previous ten (10) years; and
   2. (B) A signed authorization for a background investigation on a form prescribed by the Securities Commissioner.
