(a) Forms.

1. (1) The following forms are prescribed by the Insurance Commissioner and are attached as exhibits to this part.
2. (2) All forms are available upon request to the State Insurance Department or via the department’s website at http://www.insurance.arkansas.gov. (b) Use.

1. (1) The forms required by this part may be prescribed forms that, from time to time, may be published or amended issued by the commissioner.
2. (2) The commissioner may accept, in lieu of forms published in this part, the most recent editions of comparable forms published by the National Association of Insurance Commissioners (www.naic.org) or the SERFF Consortium (www.serff.com/).
3. (3) Although it is strongly urged that companies use the published forms, any form containing all the information required to be furnished in the published forms is acceptable if approved by the department.
4. (4) Companies are also strongly urged to submit the forms through the System for Electronic Rate and Form Filings (SERFF), by email, on a compact disc, or other desktop computer readable medium.
