(b) Benefits may not be more restrictive than the following:

1. (1) Forty-five dollars ($45.00) per day from the first day;
2. (2) Benefit period not less than three hundred sixty-five (365) days; and

3. (3) Requirement that insured:

   1. (A) Be hospitalized for at least three (3) days; and
   2. (B) Enter the facility within fourteen (14) days following hospital discharge.
