This rule shall apply to all reports, statements, and other documents filed with the Insurance Commissioner or issued to the public in relation to the business of insurance by:

1. (1) Licensed insurers;
2. (2) Health maintenance organizations;
3. (3) Fraternal benefit societies; and
4. (4) Hospital and medical service corporations.
