The following shall accompany the final payment document for contracts:

1. (1) Contractor’s invoice;
2. (2) Certificate of final completion;
3. (3) Contractor’s release of claims;
4. (4) Consent of surety;
5. (5) Department request for final payment; and
6. (6) The design professional’s acceptance and certification letter.
