(a) Format.

1. (1) Every system shall receive the tags described in this part, in such a position as to permit convenient inspection and not hamper actuation or operation of the system.
2. (2) Tags shall be between five and one-fourth inches (5 1/4") and five and one-half inches (5 1/2”) inches in height and two and five-eighths inches (2 5/8”) in width.

3. (3) All tags:

   1. (A) Must utilize a “punch out” system for dating; and
   2. (B) May be printed and established for a period of five (5) years.

4. (4) Tags shall bear the following information:

   1. (A) Printed at the top of the tag in all capital letters in at least sixteen-point bold face type will be the following: “DO NOT REMOVE BY ORDER OF THE STATE FIRE MARSHAL.”;
   2. (B) Name, address, telephone number, and certificate number of the firm performing the service;
   3. (C) Name and license number of the firm’s inspector or RME clearly printed;
   4. (D) Signature and license number of the person performing the work;
   5. (E) Day, month, and year the work was completed;
   6. (F) Type of work performed:

(i) Installation;

1. (ii) Service;

   1. (iii) Maintenance; or

      1. (iv) Inspection;
   2. (G) Name and address of the property owner where the work was performed;
   3. (H) Description and address of the property where the work was performed; and
   4. (I) Comments/impairments shall include three (3) or more lines.
