(a) The Division of Labor shall certify that individuals meet the minimum qualifications established by 20 CAR § 882-104, including:

1. (1) The training requirement of 20 CAR § 882-105 upon submission of an application on a form approved by the division;
2. (2) Proof that the minimum qualifications have been met or the applicant qualifies for an exemption; and
3. (3) The required fees.

(b)

1. (1) Each certificate of qualification shall expire on its face three (3) years from the date of issue.
2. (2) A new certificate of qualification shall be issued upon verification from the certificate holder that refresher training in conformity with 20 CAR § 882-105 has been completed.
