(a) The following documents are used for consumer loans:

1. (1) AATAFP direct loan application;
2. (2) Credit report;
3. (3) Supporting documents (proof of income, disability, and identity) and AATAFP checklist;
4. (4) Loan data summary;
5. (5) Loan amortization schedule;
6. (6) Promissory note and security agreement;
7. (7) Notification of approval/denial;
8. (8) Disbursement letter; and
9. (9) If vehicle, notice of insurance requirements and title of vehicle with AATAFP as lienholder.
