(a)

1. (1) Enrollment information shall be obtained for each child before admission.
2. (2) Sample enrollment forms may be obtained from your childcare licensing specialist.

(b) Identifying and personal data shall include:

1. (1) Child’s:

   1. (A) Name;
   2. (B) Birthdate;
   3. (C) Home address; and
   4. (D) Telephone number;
2. (2) Name and addresses of the parents and telephone numbers where the parents can be reached while the child is in care;
3. (3) Date of enrollment in facility;
4. (4) Name, address, and phone number of persons to be contacted if parents cannot be reached;
5. (5) The caregiver shall provide a written discipline policy to parents with a copy signed by the parents and retained by the caregiver; and
6. (6) Any legal documentation that has been given to the facility by the parent or legal guardian regarding the care of the child.

(c) Medical records shall include:

1. (1) The name, address, and telephone number of the child’s physician or emergency medical care facility;
2. (2) Pertinent past medical history on the child and any change in health;

3. (3) Child’s unusual food needs such as:

   1. (A) Special formulas;
   2. (B) Diabetic diet; or
   3. (C) Food allergies;
4. (4) Notes of special problems such as allergies to medication or sunburn sensitivity or needs as indicated by the parents;

5. (5)

   1. (A) An authorized record of up-to-date immunizations or documentation of a religious, medical, or philosophical exemption from the Department of Health.
   2. (B) The caregiver shall maintain a roster of children who have not completed the minimum immunization requirements.
   3. (C) A current immunization schedule is provided as an insert in this publication;

6. (6)

   1. (A) A written record shall be made of all significant changes in the child’s physical or emotional state and accidents, incidents, or injuries, indicating the location, time of day, area, or piece of equipment where the incident occurred.
   2. (B) A copy of this shall be given to the parent on the day of occurrence; and
7. (7) Any medical documentation that has been given to the facility by the parent or legal guardian regarding the care of the child.

(d) Permissions and agreements will be signed by the parents and caregiver or caregivers:

1. (1) Consent for emergency medical care and transportation for such care which shall accompany children who are transported to and from the home;
2. (2) Other transportation permission, if any, including routine and special field trips;
3. (3) Permission to participate in water activities, if any;
4. (4) Signed statements by the parents stating who is authorized to pick up the child;
5. (5) Written permission for the facility to photograph or video tape their child, if applicable; and
6. (6) Written permission for the facility to place photos and video recordings of their child on social media or other websites, if applicable.
