(a)
- (1) All employee, child, and home records shall be kept and made available to the Child Care Licensing Unit on request.
- (2) All required records shall be maintained for three (3) years.
- (3) This includes records on children no longer enrolled.
(b)
- (1) Licensing and registration Compliance Form (DCC-521).
- (2) The caregiver shall advise parents in writing that the compliance forms are available for review upon request.
- (3) Verification of childcare liability insurance.
- (4) Fire department approval, if required by local fire department, state fire code, or requested by the licensing unit due to possible hazards.
- (5) Department of Health approval, if applicable.
- (6) Verification of annual approval by the Boiler Inspector Division of the Division of Labor.
- (7) Record of emergency drills.
- (8) Plans and procedures of emergency preparedness
- (9) Pet vaccinations
(10) Attendance records on all children to include:
- (A) The date and time of arrival and departure; and
- (B) Daily parental signatures on the sign in and out forms
- (11) Transportation rosters, if applicable.
- (12) Verification of commercial vehicle insurance coverage.
