- (3) Fire department;

- (5) Child Abuse Hotline number, 1-800-482-5964;
- (6) The physicians named by the parents;
- (7) Child Care Licensing Unit Central Office number, 501-682-2350 or toll-free 1-800-445-3316; and
- (8) Home and business numbers of the parents.
