(a)
- (1) Specialized evaluation services relate to the performance of evaluations and assessments necessary for diagnostic purposes to assist the family in understanding their child’s unique strengths and needs in order to promote their child’s early learning, participation, and development and, or, or both to support the IFSP team in developing and implementing the IFSP.
- (2) Specialized evaluation services do not include evaluations related to occupational therapy, developmental therapy, speech-language pathology, physical therapy, or vision services.
(b) A local provider program must maintain the following documentation for each specialized evaluation conducted:
- (1) The date and beginning and ending time for each specialized evaluation;
- (2) The name of the parent and other caregivers who participated in the specialized evaluation;
- (3) The name and credentials of the individual conducting the specialized evaluation and, if the individual is not credentialed, the experience or other knowledge that qualifies them to conduct the specialized evaluation; and
- (4) The diagnostic purpose of the specialized evaluation and how it will assist the IFSP team in developing and implementing the child’s IFSP.
