(a) A provider must:

1. (1) Submit all reports of the following events within one (1) hour of the event:

   1. (A) Death of a beneficiary;
   2. (B) Serious injury to a beneficiary; and
   3. (C) Any incident that a provider should reasonably know might be of interest to the public or media.
2. (2) Submit reports of all other incidents within forty-eight (48) hours of the event or, if later, the next business day.
