1. (a) The worker will verify that the child or children are living with the parent or other relative.

2. (b) Acceptable methods of verification include:

   1. (1) Collateral Statement, Form DCO-76, completed by a friend or neighbor showing the child as a household member (primary type);
   2. (2) Phone contact with a friend or neighbor;
   3. (3) Information from current school records; and
   4. (4) Other types of collateral contact.
3. (c) The verification used will be documented or filed in the case record.
