The following should be considered in every case in which the good cause determination is based in whole or in part upon the anticipation of emotional harm to the child, the mother, or the caretaker relative:

1. (1) Present emotional state of the person subject to emotional harm;
2. (2) Emotional health history of that person;
3. (3) Intensity and probable duration of upset;
4. (4) Degree of cooperation to be required; and
5. (5) The extent of the involvement of the child in the paternity establishment or support enforcement activities to be undertaken.
