The program shall ensure that the following services are provided to the clients:

1. (1) Arranging and facilitating for the provision of all services as documented in the treatment plan;
2. (2) Holding regular and as-needed meetings with the client to monitor and reevaluate the individualized comprehensive plan;
3. (3) Holding regular and as-needed meetings with the program staff and others involved in the delivery of services to the client to monitor and evaluate progress;
4. (4) Maintaining records of other documentation of all services delivered to the client; and
5. (5) Developing an aftercare plan with the client prior to discharge.
