(a) A program plan will be developed and approved by the governing body which addresses outcome measures and includes:

1. (1) A written statement of the substance abuse treatment program goals and objectives;
2. (2) A written plan for implementation of the goals and objectives; and

3. (3) An organizational chart that includes the structure including:

   1. (A) Lines of:

(i) Authority;

1. (ii) Responsibility; and

   1. (iii) Communication; and
   2. (B) Staff assignments.

(b)

1. (1) The governing body will evaluate the plan annually based on the goals and objectives of the program.

2. (2) This includes operational definitions of the criteria to be applied in determining achievements of established goals, objectives, and a mechanism for:

   1. (A) Assessing the progress toward attainment of the goals;
   2. (B) Documenting program achievements not related to original goals;
   3. (C) Assessing the effective utilization of staff and program resources;
   4. (D) Documenting verification of the implementation of the evaluation plan; and
   5. (E) Identifying the results of the evaluation process.
