1. (a) The SRT shall make a formal on-site review.

2. (b)

   1. (1) Minimally, the Office of Alcohol and Drug Abuse Prevention shall inspect the facilities prior to the expiration of the program's license.
   2. (2) The office may extend a program's license for no longer than six (6) months.

3. (c)

   1. (1) The licensure review will include:

      1. (A) Examination of program documents and records;
      2. (B) Client case records;
      3. (C) Fiscal audits;
      4. (D) Interviews with staff and clients in accordance with confidentiality laws; and
      5. (E) Interviews with various community agencies/individuals.

   2. (2)

      1. (A) Other sources may be used to determine compliance as applicable.
      2. (B) The office reserves the right to contact former clients of the program under review to determine compliance with applicable standards.

4. (d)

   1. (1) Prior to the exit interview, there will be a meeting of the SRT members.

   2. (2)

      1. (A) During the meeting, each member will present his or her findings and recommendations on the area or areas assigned to him or her.
      2. (B) All areas in terms of strengths, weaknesses, or deficiencies, as well as the decision of compliance on each applicable standard, will be discussed and evaluated.

5. (e) Exit interview.

   1. (1) Following the SRT meeting, the SRT will meet with the Chief Executive Officer, Program Director or Clinical Director, and at least one (1) member of the governing board, if applicable.
   2. (2) The team members will present the review findings.
   3. (3) The purpose of this meeting will be to discuss and clarify the findings and recommendations noted by the team members.
   4. (4) The Director of the Office of Alcohol and Drug Abuse Prevention will make the final determination as to whether licensure will be granted.
