1. (a) Any adverse actions against a participant shall be issued to the participant in writing stating the:

   1. (1) Reason or reasons for the determination;
   2. (2) Participant’s right to appeal; and
   3. (3) Procedures to follow in the appeal process.

2. (b)

   1. (1) The subgrantee shall develop complaint procedures and an appeal process to resolve any issue arising between the host agency and a participant or applicant.
   2. (2) A copy of these procedures is provided to applicants, participants, and employees.

   3. (3) Procedures shall, at a minimum, provide the following:

      1. (A) The opportunity for an informal conference and immediate resolution at the lowest level possible; and
      2. (B) Formal procedures for filing a written complaint for review by the subgrantee.
