(a) Contracts awarded for the acquisition, building, or renovation of multipurpose senior centers must contain at a minimum the following:

1. (1) Rationale for request showing need for the facility and benefits to the elderly, especially those with the greatest economic or social need;
2. (2) Drawing of the original facility;

3. (3)

   1. (A) Drawing of the proposed changes in the facility.
   2. (B) A contract for expansion must be for an extension of the original facility;
4. (4) Expected life of the facility as a multipurpose senior center;
5. (5) Services to be offered in the facility;
6. (6) Correct percentage and type of match provided;
7. (7) Estimated budget summary; and
8. (8) Narrative outlining a plan of action pertaining to the scope and detail of how the proposed work will be accomplished.
