1. (a) Division of Aging, Adult, and Behavioral Health Services of the Department of Human Services staff who conducts the assessment will prepare a complete assessment report using the appropriate area agency on aging assessment instrument.

2. (b)

   1. (1) The assessment report will:

      1. (A) Identify areas worthy of commendation;
      2. (B) Any areas of noncompliance;
      3. (C) Corrective action to be taken by the area agency on aging; and
      4. (D) Deadlines for completion of corrective action.
   2. (2) Reports will be completed within thirty (30) days of the assessment visit and forwarded to the area agency on aging director.
