(a) A report by telephone or in writing shall be promptly submitted to the administrator, Certification Division, and Office of Long-Term Care, in all cases of:
- (1) Fire;
- (2) Damage to the facility due to any natural disaster;
- (3) Major incidents;
- (4) Any change in administrator or Director of Nursing Services;
- (5) Change in ownership;
- (6) All cases of reportable disease; and
- (7) Any other unusual occurrence which threatens the welfare, safety, or health of residents or personnel.
- (b) Telephone reports should be followed-up by a written confirmation.
