(a)

1. (1) A record shall be kept in a bound ledger book with consecutively numbered pages of all controlled drugs procured and administered.

2. (2) This record shall contain on each separate page the following:

   1. (A) Name, strength, and quantity of drug received;
   2. (B) Date received;
   3. (C) Client's name;
   4. (D) Prescribing physician;
   5. (E) Name of pharmacy;
   6. (F) Date and time of dosage given;
   7. (G) Quantity of drug remaining; and
   8. (H) Signature of person administering the drug.

(b)

1. (1) The person responsible for entering the controlled drug into the bound ledger should be the same person who signs for it in the drug ordering and receiving record.
2. (2) This record shall be retained by the facility as a permanent record and be readily available.
