Each facility shall have the following responsibilities:

1. (1) To provide to its residents a comprehensive program of care which:

   1. (A) Conforms fully with this part;
   2. (B) Meets the needs of the resident;
   3. (C) Provides M1 protection of residents’ rights; and
   4. (D) Promotes the social, physical, and mental well-being of residents;
2. (2) Subject to the residents’ right to privacy, to allow the Office of Long-Term Care full access to residents and the facility’s grounds and buildings at any time;
3. (3) To allow personnel from the office full access to the facility, books, and records at all times; and

4. (4) To notify the office immediately of any extraordinary event or set of circumstances which:

   1. (A) Has resulted in serious injury to a resident; or
   2. (B) Constitutes a continuing threat of injury to a resident.
