1. (1) Hospital discharge planners may coordinate with nursing facilities by assisting in:

   1. (A) Completing the DHS-703, DMS-787, and Form DMS-780 if applicable; and
   2. (B) Providing needed information, such as a comprehensive transfer sheet listing:

(i) Medications;

1. (ii) Treatments;

   1. (iii) Laboratory and x-ray, where appropriate; and

      1. (iv) Functional abilities.
2. (2) Hospital discharge planners assisting with nursing home medical necessity activities will follow the same procedures specified for nursing facilities.
