- (a) Hospital discharge planners should make referrals of hospitalized patients to the nursing facility as soon as it is anticipated that placement in a Medicaid-certified nursing facility is likely.
(b)
(1) Hospital discharge planners may coordinate with nursing facilities by assisting in:
- (A) Completing the DHS-703, DMS-787, and Form DMS-780 if applicable; and
- (B) Providing needed information, such as a comprehensive transfer sheet listing:
(i) Medications;
(ii) Treatments;
(iii) Laboratory and x-ray, where appropriate; and
- (iv) Functional abilities.
- (2) Hospital discharge planners assisting with nursing home medical necessity activities will follow the same procedures specified for nursing facilities.
