(a)
- (1) If home health services are provided in a facility by a certified home health agency, the facility shall, as part of the resident's record, maintain a copy of all home health care plans, notes, and other documentation furnished by the home health agency.
- (2) The facility shall request a copy of the foregoing from the home health agency.
(b)
- (1) Residents who require frequent skilled nursing services on a continuing basis will be assessed by the Office of Long-Term Care to determine whether the resident requires nursing home placement.
- (2) The resident's personal physician may offer a statement to the office for consideration regarding the resident's need for placement in a nursing facility.
