1. (a) Ceiling.

   1. (1) Kitchens, and other rooms where food and drink are prepared shall have a smooth, nonperforated surface that is washable.
   2. (2) Wallpaper shall not be used.

2. (b) Walls.

   1. (1) The walls of the facility shall be a smooth surface with painted or equally washable finish.

   2. (2)

      1. (A) They shall be without cracks, and in conjunction with floors, shall be waterproof and free from spaces which may harbor ants and roaches.
      2. (B) The walls in the examining room and treatment room shall have waterproof paint.
   3. (3) All walls shall be kept clean and in good repair.

3. (c) Floors.

   1. (1)

      1. (A) All floor surfaces throughout the building shall provide a surface or finish which is:

4. (i) Smooth;

   1. (ii) Waterproof;

      1. (iii) Grease proof; and

         1. (iv) Resistant to heavy wear.
      2. (B) Safety devices shall be provided on ramps.

      3. (C)

         1. (i) All floors in baths, toilets, lavatories, beneath kitchen dish washing facilities, and bedpan rooms shall have a floor covering of a continuous type.
         2. (ii) No cracks or joints in the floor covering shall be permitted in these rooms.

      4. (D) Carpet is permitted as floor covering for the following areas, provided the carpet meets the following requirements: The carpet has a flame spread rating of seventy-five (75) or less, has a smoke density of one hundred (100) or less, when the carpet is treated in accordance with NFPA 253, Flooring Radiant Panel Test:

         1. (i) Offices;
         2. (ii) Corridors;
         3. (iii) Chapels; and
         4. (iv) Day rooms.

   2. (2)

      1. (A) No pad will be permitted under the carpet.
      2. (B) The carpet is to be glued directly to the floor.
      3. (C) Prior approval by the Division of Provider Services and Quality Assurance is required before the carpet is installed.
      4. (D) In nursing homes where carpet is installed, the home must furnish equipment and have written cleaning procedures to clean and maintain the carpet.
      5. (E) This equipment must include, as a minimum, a shampooer and wet/dry vacuum.

   3. (3)

      1. (A) Facilities presently having carpets in areas other than those listed above may keep that carpet as long as it is maintained properly and free of odors.
      2. (B) If not properly maintained and free of odors, the carpet will be removed and replaced with a hard smooth surface.
