(a) The registration issued by the Department of Health to conduct procedures with controlled substances may be suspended or revoked for the following reasons:

1. (1) The registrant has violated any provisions of this part;
2. (2) The registrant has furnished false or fraudulent material or information in the application or renewal for registration;
3. (3) The registrant has been convicted of a felony under any state or federal law relating to controlled substances;
4. (4) The registrant has had his or her federal registration to handle controlled substances suspended or revoked; and
5. (5) The registrant failed to renew his or her registration within sixty (60) days after the registration expired.
