(a) Licensed training programs shall maintain and make available to the United States Environmental Protection Agency and/or the Department of Health, if requested, the following records:

1. (1) Official academic transcripts or diplomas which are evidence of the training manager and/or principal instructor meeting the education requirements set forth in 20 CAR § 130-302;
2. (2) Resumes, letters of reference, or affidavits documenting work experience, as evidence of the training manager and/or principal instructor meeting the work experience requirements set forth in 20 CAR § 130-302;
3. (3) Certificates from train-the-trainer courses and lead-specific training courses, as evidence of the training manager and/or principal instructor meeting the training requirements set forth in 20 CAR § 130-302;
4. (4) Current curriculum/course materials and documents reflecting any changes made to current curriculum/course materials;
5. (5) The course test blueprint;

6. (6) Information regarding how the hands-on assessment is conducted including, but not limited to:

   1. (A) Who conducts the assessment;
   2. (B) How the skills are graded;
   3. (C) What facilities are used; and
   4. (D) The pass/fail rate;

7. (7) The quality control plan as set forth in 20 CAR § 130-302(c)(1) which provides the training manager shall maintain the following documents:

   1. (A) Procedures for periodic revision of training materials and course test to reflect innovations in the field; and
   2. (B) Procedures for the training manager’s annual review of principal instructor competency; and
8. (8) Results of the students’ hands-on skills assessment and course tests, and a record of each student’s course completion certificate, and any other document which is not specifically listed above, but which was submitted to the department as part of the program’s application for licensing.
