1. (a) All licensed health care facilities and providers including, but not limited to, hospitals, pathology laboratories, health care practitioners, radiation treatment facilities, specialty clinics (e.g., dermatology, oncology, urology clinics, etc.), surgery centers/clinics, and dental offices shall participate in the program.

2. (b) All participants shall:

   1. (1) Designate a specific staff member or members to be responsible for reporting required cancer data; and
   2. (2) Notify the ACCR of the name or names, title, work telephone number, and email address of the designated staff member or members.

Codification Notes: "ACCR" means Arkansas Central Cancer Registry.
