(a)

1. (1) Notifiable diseases and conditions are to be reported within twenty-four (24) hours of diagnosis to the Department of Health as provided in 20 CAR § 102-104(d).

2. (2) Reports should include:

   1. (A) The reporter's:

(i) Name;

1. (ii) Location; and

   1. (iii) Phone number;
   2. (B) The name of the disease reported and the onset date;

   3. (C)

      1. (i) The patient's:

1. (c) (c) Address including county of residence;

   1. (d) (d) Phone number;

(g) (g) Race.

1. (ii) Please spell the patient's name;

   1. (D) The attending physician's:

      1. (i) Name;
      2. (ii) Location; and
      3. (iii) Phone number;
   2. (E) Any treatment information, if known; and
   3. (F) Any pertinent laboratory or other information used in the diagnosis.

1. (1) Additional disease-specific information may be requested.

2. (2) Any person desiring to further discuss reportable diseases may call the Division of Epidemiology at:
