- (a) An annual volume of at least two hundred fifty (250) admitted pediatric patients (less than or equal to fifteen (≤15) years of age).
(b)
- (1) The hospital seeking pediatric Level I designation shall meet all the criteria listed for an adult Level I facility.
(2) In addition, a hospital seeking a Level I pediatric trauma center designation shall have a:
- (A) Pediatric trauma service organized and run by a pediatric surgeon;
- (B) Pediatric ED with appropriate personnel, equipment, and facilities;
- (C) Pediatric ICU with pediatric surgery and other surgical, medical, and nursing personnel and equipment needed to care for the injured child;
- (D) Pediatric surgeon credentialed in trauma care immediately available and present in the OR for any and all operative procedures; and
- (E) General surgical resident at a minimum PGY 3 or higher resident level may initiate resuscitative care until the attending pediatric surgeon arrives.
(c) A Level I pediatric trauma center shall have, at a minimum, the following specialists active on the trauma call panel and involved in the Trauma Peer Review Committee and the Trauma Program Operations Review Committee:
- (1) Two (2) pediatric surgeons;
- (2) One (1) pediatric orthopedist;
- (3) One (1) pediatric neurosurgeon;
- (4) One (1) pediatric anesthesiologist;
- (5) Two (2) pediatric intensivists;
- (6) Two (2) pediatric EM physicians;
- (7) One (1) pediatric radiologist;
- (8) One (1) pediatric rehabilitation specialist; and
- (9) One (1) child abuse specialist.
(d) A Level I pediatric trauma center shall also have the following:
- (1) A pediatric TPM;
- (2) A trauma registrar;
- (3) A separate and distinct trauma QI program;
- (4) Pediatric-specific clinical care guidelines and pathways;
- (5) A pediatric-specific massive transfusion protocol;
- (6) A pediatric-specific IVP program;
- (7) A child abuse assessment team;
- (8) Specific credentialing by the hospital with the agreement of the TMD for any physician serving on the trauma call panel not board-certified/board-eligible in a pediatric subspeciality; and
- (9) A pediatric trauma research program with peer-reviewed publications.
