20 CAR § 54-109 – Use of multipage claims

(a) All data except revenue code and charge fields should be duplicated on successive records.
(b) All available revenue and charge fields should be completely filled before using additional records.
(c) The 0001 revenue code:
1. (1) Should be the last entry on the last record for a multi-page claim; and
2. (2) Charge should be equal to the total charge for all pages.
