- (a) Commercial medical waste treatment, storage, and/or disposal (TSD) facilities and commercial mobile treatment systems shall submit to the Department of Health an annual summary report of their activities from January 1 to December 31 of each year.
- (b) The report shall be submitted no later than March 1 of the year following the end of the reporting period.
(c) The report shall include the following information on forms provided by the department:
- (1) Name or names and address or addresses, transportation or storage permit numbers, and the amount of waste in pounds deposited/unloaded by each transporter, storage facility, or generator that transferred/unloaded medical waste to the facility or commercial mobile system;
- (2) The amount of waste in pounds treated/disposed on-site by the facility; and
- (3) The amount of medical waste in pounds/tons stored on-site that was not treated/disposed during the reportable year.
