- (a) TSD facilities shall keep a log of their daily receipts from generators and commercial transporters of medical waste.
(b) The log shall show, at a minimum, the:
- (1) Name and address of the permitted transporter or generator;
- (2) Transporter permit number;
- (3) Date of acceptance;
- (4) Weight (pounds); and
- (5) Final date of treatment/disposal.
- (c) Manifests/tracking papers shall accompany all shipments of commercial medical waste.
(d) Log records and manifests/tracking papers shall be:
- (1) Kept on file for three (3) years; and
- (2) Made available to the Department of Health upon request.
- (e) The commercial medical waste treatment, storage, and/or disposal (TSD) facility shall provide the medical waste generator or permitted transporter, which ever delivers the medical waste, with documentation of facility's receipt of the medical waste and/or treatment/destruction after medical waste has been treated or disposed, if requested.
