(a) Records.

1. (1) Duplicate records on frozen embryos must be maintained in the laboratory and with the program director.

2. (2) Records must contain the following information:

   1. (A) Developmental stage at which frozen;
   2. (B) Freezing protocol used;
   3. (C) Recommended thawing procedures; and
   4. (D) Physical location of each embryo within the storage container.

(b) Embryo containers must be labeled with the following:

1. (1) Patient’s name;
2. (2) Patient’s identification number;
3. (3) Embryo identification number; and
4. (4) Date of cryopreservation.
