(a)
- (1) There shall be an ongoing, comprehensive quality assurance program established for each facility which will monitor and resolve any identified problems.
(2) The program will include a written plan which shall include:
- (A) Methods of evaluating all services within the facility;
- (B) The evaluation of applicable procedures; and
- (C) The evaluation of infection control for the facility.
- (b) There must be documented evidence to indicate that the facility has taken appropriate remedial action to address deficiencies identified through the quality assurance program.
(c)
- (1) There shall also be documented evidence of appropriate action taken regarding each of the problems identified through the quality assurance program.
(2) Monitoring of the problem should continue until the problem is completely resolved with documentation of:
- (A) Significant findings;
- (B) Conclusions;
- (C) Recommended actions; and
- (D) Results of actions.
