(a) All facilities for in vitro fertilization shall have an organized governing body or, in its absence, a person who shall be legally responsible for:
- (1) Maintaining quality patient care;
- (2) Establishing policies for the facility; and
- (3) Assuring the conduct of the facility.
(b) The responsible party shall ensure that the facility:
- (1) Maintains proper standards of professional work in the facility;
- (2) Formulates policies and procedures pertaining to the operation of the facility;
- (3) Credentials and appoints members to the medical staff and other authorized staff; and
- (4) Develops and maintains an ongoing, comprehensive quality assurance program which will monitor and resolve identified problems.
