(a)
- (1) A licensed agency shall file an application under oath with the Department of Health upon forms prescribed by the department prior to beginning operation of a satellite office.
(2) The department will:
- (A) Review the application; and
- (B) Issue a written approval or denial of the application.
- (b) A satellite office must provide the same full range of services that is required of the licensed parent hospice.
- (c) The governing body and administration of the parent hospice must be able to exert the supervision and control necessary to ensure that all hospice services continue to be responsive to the needs of the patient/family.
- (d) Each patient of the satellite office must be assigned to a specific IDG.
(e)
- (1) Current active patient records will be maintained by the satellite office but must be available to the state surveyors at the parent location if requested.
(2) Locations that do not meet these criteria:
- (A) Will not be approved as a satellite office; and
- (B) Must obtain a separate license.
