(a)
- (1) When a death occurs in a hospital or other institution and the death is not under the jurisdiction of the medical examiner or coroner, the person in charge of such institution, or his or her designated representative, shall be responsible for obtaining the medical certificate.
- (2) This responsibility does not include DOA in the hospital emergency room.
(b) Hospitals or other institutions may assist in preparation of the certificates by completing the following items:
- (1) Name of deceased;
- (2) Date of death; and
- (3) Place of death.
Codification Notes: "DOA" means dead on arrival.
