(a) The new certificate of birth shall:

1. (1) Be on the form in use at the time of its preparation; and

2. (2) Include the following items and such other information necessary to complete the certificate:

   1. (A) The name of the child;
   2. (B) The date and place of birth as transcribed from the original certificate;
   3. (C) The names and personal particulars of the adoptive parents or of the natural parents;
   4. (D) The name of the attendant, printed or typed;
   5. (E) The same birth number as was assigned to the original certificate; and
   6. (F) The original filing date.
