The State Registrar of Vital Records or his or her designated representative shall verify that:

1. (1) No prior birth certificate is on file for the person whose birth is to be recorded;
2. (2) He or she has reviewed the evidence submitted to establish the facts of birth; and
3. (3) The abstract of the evidence appearing on the delayed certificate of birth accurately reflects the nature and content of the document.
