(a)

1. (1) The State Registrar of Vital Records or his or her designated representative shall abstract on the delayed certificate of birth a description of each document submitted to support the facts shown on the delayed birth certificate.

2. (2) This description shall include:

   1. (A) The title or description of the document;
   2. (B) The name and address of the custodian;
   3. (C) The date of the original filing of the document being abstracted; and
   4. (D) All birth facts required by 20 CAR § 1-405 contained in each document accepted as evidence.
