(a)
- (1) The State Registrar of Vital Records or his or her designated representative shall abstract on the delayed certificate of birth a description of each document submitted to support the facts shown on the delayed birth certificate.
(2) This description shall include:
- (A) The title or description of the document;
- (B) The name and address of the custodian;
- (C) The date of the original filing of the document being abstracted; and
- (D) All birth facts required by 20 CAR § 1-405 contained in each document accepted as evidence.
- (b) All documents submitted in support of the delayed birth registration shall be returned to the applicant after review.
